HCVRS MEMBERSHIP APPLICATION
To help make the application process a little easier for you, we make our application paperwork available to you here as a fillable PDF to download and print. You can complete the form on your computer and print it or you can complete it and then save it on your computer. Once it is saved, you can attach it to an email to our Membership Committee Chair, Dawn Varner at email@example.com or you can fax it to 540-468-2296 and mark it: Attention Dawn Varner.
The DMV Record form will need to be printed and signed by you the candidate and returned to HCVRS.
